Being Mortal

In Being Mortal, surgeon and New Yorker staff writer Atul Gawande examines how modern medicine often fails the dying and the aging, and asks what it means to live well near the end of life. Drawing on his own experience as a doctor—and, later, as the son of a father with a spinal tumor—he argues that medicine is trained to fight death rather than to prepare for it, and that this bias produces suffering even when treatments "succeed." Gawande contrasts the rise of nursing homes with alternatives like assisted living, tracing the work of pioneers such as Keren Brown Wilson, who built one of the first assisted-living facilities in Oregon around residents' autonomy rather than safety. He also visits Bill Thomas's Eden Alternative, which brought plants, animals, and children into a nursing home in upstate New York to combat the "three plagues" of boredom, loneliness, and helplessness. In the book's central clinical chapters, Gawande follows patients including his daughter's piano teacher, who chose hospice over further chemotherapy, and a young woman with terminal cancer weighing quality of life against aggressive treatment. He introduces the "hard conversations" framework: asking patients what they understand about their prognosis, what their fears and goals are, what trade-offs they will accept, and what a good day looks like. The book's final section turns to Gawande's own family, describing his father's decline and the difficult choices about surgery, hospice, and where to die. Being Mortal, published in 2014, became a bestseller and helped popularize the field of palliative care.
